Complete the EPLI application or endorsement form:

  1. If your agency has less than 10 employees, EPLI coverage is added to your existing E&O policy by endorsement. Complete the endorsement form and email to Insurance Programs Director, Kate Langford, at kate@bigimd.com.
  2. If your agency has more than 10 employees, EPLI coverage is added by a separate policy.  Complete the EPLI Application and email to Insurance Programs Director, Kate Langford, at kate@bigimd.com.
